In preparation for the 2009-2010 sports season, the University of Mary athletic training staff needs to develop and update the medical files for all Marauders student-athletes. The information we are requesting on the enclosed forms enables us to provide medical care for athletes in the University of Mary athletic training room.
Students-athletes will not be allowed to participate in intercollegiate athletics at the University of Mary until all medical information, including proof of insurance, is on file in the University of Mary athletic training room.
