Application for Admissions: Master of Science in Occupational Therapy

Students are eligible to apply to the Professional Occupational Therapy Program upon meeting the admissions criteria, which can be found on the University of Mary webpage.
Students may apply to the program prior to completing all pre-requisite coursework, however all courses must be completed by August 27, 2013. All applications, including official transcripts, three professional references, volunteer verification forms and the $40 application fee are due January 7, 2013.
Students need to send all official transcripts to: University of Mary, OT Program, 7500 University Drive, Bismarck, ND 58504.
There is a $40 fee for the application process. Please include a check or money order to the University of Mary with your Volunteer Work Verification Form.
This application is in six short parts. Each part must be completed for the online application to be consdidered. Click the text link for each section to view the contents of that section. When you're done, click the "Submit" button at the bottom of the page.

You must download submit the Recommendation and Appraisal Form and Volunteer Work and Community Service Verification Form.
This information is necessary to fully consider your application to the program.
Print three (3) copies of the Recommendation and Appraisal Form and follow the directions on the form. References must be sumitted in original, sealed envelopes, with the signature of the referring individual across the seal. References submitted without required signature across the seal will not be accepted.

Part 6: Verification Information
|
The University of Mary, like other public and private colleges and universities, has a policy regarding the admission of persons with a criminal history. Having a criminal record will not automatically prevent an applicant from being admitted to a course of study at the University of Mary.
Applicants are required to answer all of the following questions. Applicants who provide affirmative responses to any question will be asked to provide additional information which will be reviewed by the Academic Standards and Admissions Committee. Falsification or omission of information may result in a denial of admission, rescission of admission, dismissal, or other appropriated action.
