   2011-12 Charge Authorization Form
  • Students must be registered for classes prior to requesting a U-Mary Bookstore Charge.
  • Students who have excess federal Title IV financial aid or excess 3rd party reimbursement can use these excess funds to pay for charges at the University of Mary Bookstore. The timeframe to charge books/supplies may be limited each semester.
  • Students who do not have excess aid available can charge at the University of Mary Bookstore. All charges will be due in full on or before the fee payment date each semester. The timeframe to charge books/supplies may be limited each semester.
  • In all instances, a charge authorization form must be on file before students will be allowed to charge to their student account. The form will be required annually.
  • Pell-eligible students: The University of Mary will allow you to charge required books and required supplies at the University of Mary Bookstore.
    • The University of Mary Bookstore Charge Authorization Form, if signed by the Pell-eligible student, gives authorization to the university to use the student's Title IV financial aid funds to pay for books and supplies.
    • If the Pell-eligible student does NOT elect to use the bookstore charge option by not charging books and supplies at the University of Mary Bookstore, the student is considered to have 'opted out' of the university's option to assist students in purchasing books and supplies. The university has no obligation to provide an alternative option.
  • Eligibility to charge will be at the discretion of the bookstore staff, regardless if an authorization has been completed or not.
This authorization form is required for ALL students requesting a charge of books/supplies to their U-Mary student account.
