Students may request the Immmunization Records by contacting Tina at the phone numbers or email address listed above.

Due to confidentiality, we may only release Immunization Records to the students themselves.

If the student was seen as a patient at the University of Mary Student Health Clinic, and they are requesting actual medical records, we must have a release of information form completed.  Please print out the Release of Information, complete it in it's entirety, and return it to the Student Health Clinic.  Please allow 7-10 business days for this type of release to be completed.

Immunization records and patient information are kept on campus for one year following the student's departure from the university.  Files are then transferred to Mid Dakota Clinic for storage.  If it has been greater than one year since the student's departure, you may contact the Mid Dakota Clinic medical records department directly at (701)530-8061 or (800)472-2113 ext. 6081.  Please ask to speak to Shirley or Babe.

The following information will be needed to process your Immunization Records request:

• Student's full name
• Student's date of birth
• Last date attended
• Complete mailing address and fax number for records to be sent to the student.

Please keep a copy of your Immunization Record for your own permanent file and photocopy as needed.  You will need to provide documentation of your immunization status throughout your entire lifetime.
