Clinic Billing Policies

Students may receive health care services from the Student Health Clinic regardless of ability to pay.
Insured Students:
If insured, the fees will first be billed to your insurance carrier, and the balance can be billed either to the insured or the student. Special arrangements may be made on a case by case basis.
Parents are encouraged to check with their insurance provider to see if an out of area waiver is necessary for coverage while student is in college. Please check the back of your insurance card to contact your insurance company's customer services office.
Some insurances require enrollment verification forms be submitted for every semester the student is enrolled to continue health care coverage. Parents are encouraged to contact their insurance company for specific guidelines. Please check the back of your insurance card to contact your insurance company's customer services office.
Uninsured Students:
If uninsured, the student will sign an insurance waiver for each clinic visit.
Confidential Visits:
Confidential visits are available for those services of such a highly confidential nature, that for these services, there will be no billing at all. There will be no billing or insurance filed for these visits.
Payment Options:
Mid Dakota Clinic mails monthly statements to the person the student specifies when they register.
Please allow the time for your insurance policy settlement to be processed and the final balance is noted on your Mid Dakota clinic statement.
The following payment options are available:
- Mail your payment in the self-addressed envelope provided.
- Bring payment to Student Health Clinic
- pay by phone with credit card by calling the 800 number listed on your Mid Dakota Clinic billing statement.
- Click here to pay online with your credit card.
