What information is in a student's counseling record?
When a student receives services at Counseling Services, a file is started for that student. Information in the file includes:

1. Information provided by the student in advance of the first appointment (name, contact information, reason for seeking services, family, social and mental health history, etc.)

2. A record of all appointments, cancelations, reschedules, and missed appointments.

3. The counselor's notes of each counseling session describing progress towards counseling goals.

4. Copies of any release of information signed by the student and a copy of information released in response to the request for release.

5. Agreements or contracts made between the student and the counselor.

6. Results of any screening instruments or testing that has been used in the course of the counseling sessions.

6. Information provided to Counseling Services from a referral source or information provided by other medical or mental health professionals in response to a request from the student.

7. Information provided by other university officials or departments. 
  
Where are student counseling records stored?
Student counseling records are stored in locked files in the Counseling Services Office.

How long are student counseling records maintained?
Student counseling records are maintained for seven years after the last appointment. After seven years, the records are destroyed unless they are a part of an ongoing legal case. 

How can I have copies of my records sent to another mental health professional? 
Students who would like information from their file released to another qualified medical or mental health professional may do so by providing a written release of information to that effect. Counseling Services has a form available for that purpose.

Students should be aware the university is not able to release information in the student's counseling file originally provided by another counselor or medical provider. The student must contact the original provider of the information to have copies of the information sent to a third party.  

The university reserves the right to charge reasonable administrative fees for expenses (copying and shipping) associated with complying with a student's request to release information. Please allow the University a minimum of 10 business days to comply with your request.

Am I able to get copies of or view my counseling records?
Students may view their own counseling records upon request. The request must be in writing. Please allow Counseling Services a minimum of 10 business days to comply with your request. Counseling Services reserves the right to refuse to furnish copies of information in the student's file to directly to the student. We will however, provide copies of any information in the file to a qualified medical or mental health professional upon receipt of a release of information as indicated above.
