The Student Teacher Evaluation Forms are designed to evaluate each student teacher's knowledge, performance (skills), and dispositions at various stages of the student teaching experience. The assigned cooperating teacher and university supervisor are required to complete the appropriate student teaching evaluation forms for his/her assigned student teacher. The university supervisor is the instructor of record and will assign the student teacher a final grade of pass or fail for each student teaching assignment.
