Evaluation Forms

The Practicum Student Evaluation Forms are designed to evaluate each practicum student to determine if he/she possesses the knowledge, performance (skills), and disposition necessary to become a competent professional teacher. The assigned practicum teacher is required to complete the practicum student evaluation for each of his/her assigned practicum students.
In addition to the practicum evaluation completed by the assigned practicum teacher, each student is required to complete a Practicum Student Self-Evaluation Form and a Practicum Teacher Evaluation Form. The practicum student self-evaluation is designed to help the student reflect on his/her practicum experience and to assess his/her strengths and weaknesses as a pre-service teacher. The practicum teacher evaluation form is designed to evaluate the effectiveness of the assigned practicum teacher.
